  • Establishing and maintaining effective relationships with residents and clients
  • Encouraging resident and client choice and participation in pastoral care services
  • Providing spiritual support and pastoral care services to residents, clients, family members, and employees that is person-centred, faith-based, inclusive, and friendly
  • Preparing spiritual celebrations to recognise special days of remembrance
  • Preparing prayers and moments of reflection to share with residents, clients, family members and employees
  • Creating and providing leadership for worship and pastoral care activities with residents, clients, and their family members when whey enter palliative care or end of life
  • Supporting employees, residents, clients, and family members when residents and clients are entering palliative care or end of life
  • Supporting the teams and resident / client family members access support services to assist with bereavement associated with the loss of a resident or client
  • Maintaining pastoral care assessments and progress notes within eCase / CRM
  • Promptly reporting any changes in resident or client health status to the Registered Nurse or Care Advisor on duty
  • Facilitating pastoral and spiritual visits and support from ministers of various faith groups
  • Coordinating liturgical resources and maintaining the reflective space of chapels and prayer rooms

  • Aged Care Quality Standard No 1 and 4 is achieved within the home
  • Strong relationships exist with external clergy and spiritual mentors
  • Resident and family feedback, as received via resident surveys, compliments and complaints

 

Mission Formation

Supporting mission formation activities for employees within the residential aged care home

 

  • Working collaboratively with the Mission Manager to provide learning and development opportunities for employees and volunteers to support the deeper understanding of the Catholic Healthcare mission and values
  • Working collaboratively with the Residential Manager or Regional Manager to provide opportunities for employees and volunteers to support the delivery of pastoral / spiritual care services to our residents and clients
  • Working closely with the Mission team to provide formation activities for employees
  • Actively participating in continuous professional development activities for yourself, including attending retreats, conferences, or short courses as offered

  • Employee feedback, as received via employee surveys, compliments and complaints
  • Evaluations and feedback from Mission / Pastoral Presentations or related mission matters made on orientation days, training days, or employee formation
